How to use the “Submit Online” feature:

It may take up to 10 minutes for the changes to reflect on your checklist. Your patience is appreciated!

Step 1: Download form, complete, print and sign

Step 2: Scan and save the document as a designated document type

  • Acceptable document types:
  • Up to files may be uploaded, and each file may be up to in size.

Step 3: Go to your checklist and click on Submit Online button to upload document

Step 4: Click on “Choose Files”, window will open, choose document and click upload

Step 5: Upload Document

Step 6: Confirmation Page; keep a copy for your records

Do not upload documents for other departments. They will not be processed.
